The North Estonia Regional Hospital Rehabilitation and Palliative Care Clinic is located in Tallinn at Hiiu 44 and Hiiu 39. The Rehabilitation and Palliative Care Clinic has four departments.
Department of nursing care - The department is located Hiiu 39. The department provides nursing care services, and as a result the department is also termed the nursing care department (previously the nursing care department).
Aftercare Centre:
- Department I - The 1nd follow-up care department is an intensive care department and is located at Hiiu 44.
- Department II - The 2rd follow-up care department provides surgical and oncological aftercare and is located at Hiiu 44.
- Department III - The 3th follow-up care department provides neurological and internal diseases aftercare and is located at Hiiu 44.
Rehabilitation Centre - The centre is located in two medical campuses of the medical centre. Outpatient rehabilitation is provided at the Mustamäe medical campus (19 J. Sütiste tee) and both outpatient and inpatient rehabilitation services are provided in the Hiiu medical campus (44 Hiiu Street).
Palliative Care Centre - The centre is located in two medical campuses of the medical centre: Mustamäe medical campus (19 J. Sütiste tee) and Hiiu medical campus (44 Hiiu Street).
The clinic has 13 doctors, 63 nurses and 59 caregiver positions, with specialists providing support services. The clinic's workers are members of professional associations and cooperative networks, and they work with many of Estonia’s central hospitals and exchange knowledge and experience at international conferences.
Cooperative projects
The clinic works with health care colleges and medical schools, serving as a practicum internship base for nursing students as well as for caregivers. The clinic has practicum supervisors who have undergone mentor training. The work methods at the clinic’s nursing and caregiver therapy personnel are based on evidence-based nursing, and as a result, new work techniques are developed and implemented at the best possible level.
What is follow-up care?
Follow-up care is treatment that immediately follows active care and is specifically tied to the active care that preceded it. The goal of follow-up care is to help restore people’s health and functional status. To restore health, in certain cases a longer hospitalization is required to enable and reinforce a full recovery.
The treatment prescribed by the attending physician is continued while in follow-up care. The patient is prescribed a suitable dietary regiment and, if necessary, physical therapy. The entire treatment process is monitored by a physician, who also organizes the patient’s subsequent care or refers the patient to other institutions pursuant to the patient’s status.
The team in various departments of the follow-up care clinic is made up of intensive care physicians, general practitioners, surgeon, social care workers, intensive care nurses, general nurses and caregivers. A Medical Centre spiritual adviser offers spiritual and non-material support.
If necessary, patients can also consult a physical therapist, occupational therapist, psychologist and social worker. The social worker helps to find answers to questions related to the need to introduce practical changes to the everyday living arrangements. Questions may come up during absences from work, in applying for aid and support (such as pension for incapacity for work and disabilities), obtaining assistive device and dealing with officialdom.
The psychologist helps regain mental balance and better organize thoughts and emotions if the illness of someone close to a person causes anxiety, stress and a feeling of helplessness. We provide psychological counselling to patients and patients’ family members and loved ones.
